THP-NMD Provider Roster

THP-NMD, formerly known as THP+FC, is a foster care placement for youth ages 18 to 21 who are participating in extended foster care (non-minor dependents). THP-NMD provides housing and a wide range of supportive services, including case management and counseling, job training and educational support. This roster lists by county, all state-licensed THP-NMD providers who are currently operating programs.
